UMPI athletics asked that all new student-athletes make an appointment with their primary care physican to perform a physical. The paperwork that is required for particpation can be downloaded at the link below. If you are unable to to make an appointment with your primary care physicain before arrival to campus in the fall we ask that you email our athletic director Dan Kane at daniel.c.kane@maine.edu.

 

For returning UMPI student athletes who have had a physical completed last year we encourage you to have a physical completed; however, it will not be a equirement for particpation. It is required that you fill out the update physical paperwork and disclose of any changes in health.

 

Please bring this paperwork with you when you come to campus in the fall.
